For the well-being and safety of our campers and to comply with Pennsylvania State laws, all GMYC Staff Members (ages 18+) must complete several background checks including a state criminal record check and a child abuse history form. Any volunteers who have not lived in PA for the last 10 consecutive years must also file for an FBI clearance. 

 

THESE FORMS MUST BE COMPLETED PRIOR TO SERVING AS A STAFF MEMBER AT GMYC!  It may take several weeks to complete some of these clearances - DO NOT WAIT TILL THE LAST MINUTE!

 

If you have already received these clearances WITHIN THE PAST 5 YEARS for ministry in your home church or volunteering at other organizations, or even for your employment, you can request a copy and send it to us.  All clearances will have to be renewed 5 years after their original date of issue.

 

If you do not already have your clearances, please follow these steps to complete the process:

 

1. Please print, read, and sign the GMYC Volunteer Guidelines. 

You will find the guidelines here.  Please return a signed copy to GMYC.  Feel free to keep a copy for your own reference.

 

 

2. Complete the free PA State Police Criminal Background Check. 

This can be done online by going to https://epatch.state.pa.us.  It only takes a few minutes, and you will have your results in minutes.  However, if you prefer, you can download it here, print it out and return it by mail. The form states that you need to send $10, but that is no longer true as of July 25, 2015. Send the form without payment.

Please return this background check to GMYC.

 

 

3. Complete the free PA Child Abuse History Clearance. 

This can be done online by going to https://www.compass.state.pa.us/CWIS.  This will require you to create a user name and password, but it is still the simplest and quickest way to complete this background check (it may still take several weeks to receive your clearance).  You can also download it here, print, complete, and mail.

Please return this clearance to GMYC.

 

 

4. If you have lived in PA for the last 10 consecutive years or more, complete an affidavit stating that fact.

It can be found here.  Please sign and return to GMYC.

 

5. If you have not lived in PA for the last 10 consecutive years, you must be fingerprinted.

Go to Identogo and follow the instructions.

 

If you complete these clearances online, they will come back much more quickly (State Police one will be issued immediately; the second one may take several weeks). Please make a copy of each one and get that to us and any other organization in PA where you volunteer.

When you receive your processed clearances, please return them to GMYC as soon as possible.  Scanning and emailing them to us will be the quickest by far, but if you must mail them, that is acceptable also.
